First meeting
Initial Session
For your first session, please click on, print, fill out, and bring the following forms:
Client Information Form
Informed Consent (read and signed by all participants)
If you have the forms filled out, please come to the office at your scheduled appointment
time. If you could not bring them with you, please arrive 10-15 minutes early to fill them out.
Let me know so that I can have the forms waiting for you on a clipboard on the office door.
The entrance is around the back of the building. Please park on the street or in the
assigned client parking spot You'll see my name plate mail box in the back and on the
door. Go up the stairs and my office is the first one on the left. If the sign says, "In
Session" please wait outside until your session time. If the sign says, "Please Knock," do
so and I will be right with you.
There is a bathroom at the top of the stairs second door on your right.
When you session begins, we will go over your paperwork together, make sure you
understand the informed consent, and then you can ask me any questions you have and
we discuss what brought you in. At the end, we will reschedule for the following week.
It is okay to call me between sessions with questions. Please give 24-hour notice to cancel
or if you need to reschedule.
